If the full payment is not received by January 31st, 2013 the Tournament Committee can not guarantee the entry to the Main Draw of the Tournament. Registrations without the full payment will be placed on the waiting list. If you pay with Pay-Pal, you will be placed in the Tournament's Main Draw. However you will be confirmed in the Main Draw immediately after we acknowledge the receipt of payment with Pay-Pal. The processing fee of $15 will be added to all Pay-Pal transactions for Player's enter fee and $22 for Player and Guest.

I agree to Polonia Open 2012 by Zywiec Golf Tournament regulations. I agree and fully understand that there are certain risks inherent in the game of golf and accept personal and sole liability for all personal injuries and damages. I agree and consent to have my picture taken during the tournament and allow the organizers to have it published. By signed this application, I waive all my rights to bring suit against Polonia Open 2012 by Zywiec, PAGA, all event sponsors, event organizers, event committee members, officers and the Fountains Country Club.
I agree that this entry is subject to rejection at any time including the Polonia Open 2012 by Zywiec tournament. The committee will be the sole judge of rejecting an application or reject a participant from or during the tournament.
